Here are some of my favourite recommendations for your trip to Italy. Whether it's your first time, or third, there is always something to see. Here are some of my must sees in terms of sight seeing, restaurants, and cities to day trip to.

I would recommend flying open jaw concept, which means you fly into one city, and fly out of another. This will make things easier when flying out, and you will have to travel less of a distance when leaving the country to head back home.
Transportation: In terms of renting a car, I don't recommend it in Rome or Florence, however if you want to make day trips when you are in Tuscany, definitely do so. Just be careful when driving in Florence, because driving in the city center is limited to locals only because of the historic center. You will get a ticket, and they are very expensive. Rome is also a bit crazy to drive in, so just keep that in mind as well. Driving in the countryside however is recommended as its the best way to get around in areas such as Amalfi, and Tuscany if you want to visit multiple cities. If you decide against renting a car when travelling between these cities, I highly recommend using Trenitalia for excellent high speed service. Make sure you look up train station names, as they differ from the local city airport. For example, in Rome there is Termini station which is the main station in the city center. However, there is also a station at the airport Fiumicino. Just ensure you are purchasing tickets for the correct station!
Museums & Sightseeing: I highly recommend booking things in advance and purchasing tickets for major sights such as the Vatican Museum, The Colosseum, and other art galleries.
Accommodations: I love booking Airbnb rentals for an authentic Italian experience. I've actually never stayed at a hotel in Italy before, and have solely done vacation rentals and Airbnbs. Make sure you read reviews, stay in good locations, and make sure you have the amenities you need. I would recommend comparing hotel prices and Airbnb to see what fits best for you.
Food: Always try to eat away from main tourist attractions, and read reviews. Usually any Trattoria you pick will be amazing, but a lot of the tourist trap restaurants are located directly near museums and attractions. Try to venture off onto side streets to find better food.
Cities to Visit
Rome- I recommend at least 3 nights in Rome to soak it all up if you like history, nightlife, and a busy metropolitan town.

Trastavere Must eats: Roscioli, Gelateria Frigidarium, Mimi e Coco, Emma, Pizzeria da Baffetto, Taverna Lucifero, 443 Restaurant, Le Montecarlo
Tuscany/Florence- I reccomend at least 2 nights in Florence, and 2-3 in Tuscany, or do 3-4 in Florence and day trip from there with the train or car. Be careful if renting a car in the Firenze city center, I would not recommend it.


Florence- Firenze is known for its famous Renaissance art, as it was the city that birthed Renaissance art, sculptures, and architecture.
Uffizi Gallery (the perfect spot if you love Renaissance art)

Galleria dell' Accademia (where the statue of David is)

Palazzo Vecchio
Via Tournabuoni- if you like to shop at luxury stores and boutiques, this is the street for you.
Duomo- the Cattedrale di Santa Maria del Fiore is an iconic spot which helped set the tone of the Italian Renaissance period. An incredible structure and must see.
Must eats: 13 Gobbi, Trattoria Lo Stracotto, La Spada, Trattoria Marione, Don Nino Sicilian Cannoli Shop
Tuscan Day Trips
Lucca Pisa Siena
San Gimignano (my favourite town in all of Tuscany) Activities
Wine Tour- highly recommend Grape Tours they tour from Florence, and offer transportation, lunch and tastings for two Tuscan wineries and an olive oil production.
Cooking Class- I recomend Montese Cooking School with Chef Mauro. Fantastic experience
Tenuta Torciano- wonderful winery and immersive experience for tasting
The Mall - if you like shopping and discounted Prada, Gucci and Italian designers, you have to check out The Mall which is about 40 minutes outside of Florence. Incredible deals!
Must eats: La Grotta di Fulignano
Amalfi Coast - 4 nights would be a perfect way to wind down.

Capri- try to stay over night, as day tripping can be a bit hectic and its worth the stay over. Make sure you check the ferry schedule.
Positano
Ravello
Amalfi
Must eats: Ristorante Max in Positano, Vittoria in Ravello, L'Abside in Amalfi, Bianca Zita in Furore, Restaurant da Paolino in Capri
Venice- 1-2 nights would be just fine. I was not a huge fan of Venice, but I do believe that everyone should visit once in their lifetime for the experience. It is my least favourite of the Italian cities, but nevertheless is still a beautiful experience.
